UESF members who have Kaiser should be aware that it is Kaiser's policy not to let individual physicians sign the required document. In order to get the authorizing signature, Kaiser members should take the completed form to the Kaiser member benefits office at 2238 Geary Boulevard, second floor.

Paid Family Leave Insurance - For Paraprofessionals, and other California workers, covered by State Disability Insurance, Paid Family Leave (PFL) insurance provides up to six weeks of benefits for individuals who must take time off to care for a seriously ill child, spouse, parent, or registered domestic partner, or to bond with a new minor child.
